This is the structure we use when assembling board-ready audit reporting for App Clouds clients across the UK.
Findings in severity order
Each finding states the issue, why it matters to the board, the agreed rating, and the named owner. Age of open items sits beside the rating so drift is visible.
Management responses
Concrete actions with dates—not aspirations. Where management disagrees with a rating, the disagreement is recorded without burying the finding.
Evidence appendix
Indexed excerpts, sampling notes, and limitation statements. Directors who want depth can find it; those who do not are not forced through it.
When to request pack assembly
- Fieldwork is substantially complete
- The committee date is three to six weeks away
- Severity ratings are still being debated
- Last year’s pack drew complaints about length or clarity
